Emergency Contacts
We are encouraging all members to supply us with emergency contact information, although this will not be compulsory. The emergency contact information will be stored on forms in a ring binder that will be kept in the RWBSA Office, which will be locked when not in use.

You can get a copy of the Emergency Contact Form by clicking on the button below. If you choose to fill it in, at a minimum you should include your full name and one emergency contact. You can also include a second emergency contact and any medical information that you believe would be helpful if you were unable to provide this at the time of any incident. In addition, your date of birth along with your full name would assist medical personnel in accessing your medical records. If you choose to provide any information, you can either download the form or get a paper copy that is available in the clubhouse. Please give the completed form to the Welfare Officer.

Even though we do not at this stage intend to store any of this information digitally, it will still be governed by GDPR regulations. We have therefore made available a privacy notice that will ensure that the club complies with GDPR. You can view this by clicking on the button below.
